What sorts of things makes up "nothing out of the ordinary"? Before I switched my setup around to use a Pi5 with a NVMe, I had cabling for a SATA SSD and was concerned about the strain the heavy duty USB/SATA cable put on the socket. However, I would assume that strain would break the solder connections rather than rip out the blue keying insert. Do you see any patterns with which USB devices seem to remove the keying insert and have you inspected them under magnification?Nothing out of the ordinary ! That's why I wondered if it is a know problem.
I've been plugging things into USB for as long as USB has been around (nearly 30 years) and I've never had this happen before.
The revised setup has the NVMe SSD mounted under the Pi PCB, with an externally powered USB3 extender on the other USB3 port for "removable media" things. Mouse and keyboard live on the KVM switch so that I never plug things into the ports on the Pi directly anymore.
I have an OS testbench Pi5, that sees more USB plug action, that has not had any issued to date.
