There is always a reason.Setup: RPi 400 running Bookworm and LXDE, all fully updated.
Suddenly, with no apparent reason (other than, maybe, I have installed Timidity) I have lost the Speaker icon off the Volume Icon (PulseAudio) applet on the LXDE Panel, but the Mic icon is still showing. When I install the Volume Icon applet in addition and run alsamixer in a terminal I can see the Volume Icon slider controlling the alsamixer graphic. More concerning is when I run sudo raspi-config in a terminal and do System->Audio it immediately flashes back to the System menu.
I'm running out of ideas...
Something must have changed.
In order to get the icon back, you just right click on the taskbar, select "add or remove plugins", then select "volume" and "add to left" or "add to right" It should re appear.
On your second issue...What changed; seems to have been the installation of timidity..
How did you install it?
