Are you measuring the correct pin? Pin 1 is on the inside row of 20 pins. The pin nearest the board corner is pin 2 and should be 5 Volts nominal.And whats worrying is that pin 1 reads 4.8~4.9V.
Remove power, remove everything from USB sockets and GPIO header. Remove microSD card. Have HDMI connected to display. Apply power. Do you get any output from the RPi4B on the display?
